Abstract:

This study is aimed to analyze the performance of metal stamping process in producing frame top used for oral care product. Historical data of 2-month-production activities was analyzed in order to determine the breakdown of reject contributors with use of Pareto diagram. Going through with Six Sigma Method: Define, Measure, Analyze, Improve (DMAI) then analysis of process capability, root cause contributing to the major defects, and prioritized improvement according to Risk Priority Number (RPN) in Failure Mode and Effect Analysis (FMEA) was demonstrated in a quantitative manner. This quality evaluation resulted in the Sigma level of the process was of 3.56 or process capability index (Cpk) of 1.18. The most significant factor found to produce defects in the process was manpower (operators) who were responsible either in machine operation or product measurement. Implementation of strict Standard Operating Procedure (SOP) and certified operator system will result in a clear system specifically, on who will take responsibility on any failure occurred during the operation. Furthermore, this system will effectively suppress the index of frequency and detection factor in order to reduce risk of failure modes.
